COVID-19 Self-Reporting Guide


Reporting on COVID-19 symptoms, exposure and testing is essential to the health and safety of our community. The following guide can help you determine whether to self-report.

Note that if you answer 'yes' to any of the questions below, you must self-report your status, even if you are fully vaccinated and/or boosted.

 

Do you have any of the following COVID-19 symptoms?

  • Fatigue
  • Muscle or body aches
  • Sore throat
  • New loss of taste or smell
  • Congestion or runny nose
  • Nausea or vomiting
  • Diarrhea
  • Headache
  • Fever over 100.4°F or 38°C
  • Chills
  • Cough
  • Shortness of breath

If Yes:

  • Self-Report Now
  • Go home immediately and isolate
  • Seek a COVID-19 test
  • Await guidance from the COVID-19 Team

If No:

  • Continue reading below.

Have you been in direct or close contact* with a confirmed COVID-19 case?

*Direct or close contact is defined as less than 6 feet apart and more than 15 minutes of contact with a person who has tested positive for COVID-19.

If Yes:

  • Self-Report Now
  • If you are up-to-date* on your COVID-19 vaccinations:
    Up-to-date is defined as having received all recommended COVID-19 vaccines, including any booster dose(s) when eligible.
    • Self report your up to date vaccination status. If you have NO symptoms, and NO positive COVID-19 diagnosis, you will be cleared to return to campus.
  • If you are NOT update-to-date on your COVID-19 vaccinations:
    • Go home immediately and quarantine
    • Await guidance from the COVID-19 Team

If No:

  • Continue reading below.

    Have you received a positive COVID-19 test result in last 14 days, or since the onset of symptoms?

    If Yes:

    • Self-Report Now
    • Go home immediately and isolate
    • If you tested positive over 10 days ago and you are feeling better, you may return to campus
    • Await further instruction from the COVID-19 Team and/or the Denver Department of Public Health and Environment

    If No:

    • No need to self-report, or isolate.

    Please note that the self-reporting form is NOT the same as the vaccine verification and exemption form and should not be used to self-report your vaccination status. 

    After submitting your self-report - if you do not receive an auto-clearance email allowing you to be on campus - let your supervisor/professor know and stay home until contacted with further instructions. Follow the recommendations of the CU Denver Contact Tracing Team, public health agency and/or medical provider. Please do not return to on-campus activity until the recommended guidance return date and when you are feeling better.
